The warning comes amid persistent difficulties tracking chains of transmission in affected provinces. WHO cautioned that limited surveillance and delayed case detection are impeding efforts to measure the outbreak accurately.

Majority of new infections not linked to known cases

WHO officials reported that roughly 80 percent of recently identified infections have no known epidemiological link to previously confirmed patients. This pattern signals widespread transmission within communities and suggests that many infections are either asymptomatic or unreported.

Unlinked cases complicate contact tracing and isolation efforts, reducing the effectiveness of standard outbreak control strategies. Health authorities are therefore prioritizing expanded testing and community engagement to identify hidden transmission networks.

Government figures show concentrated burden in Ituri

DRC government data states that 1,792 cases have been recorded to date, with 625 confirmed deaths, underscoring the severity of the outbreak. The government says approximately 90 percent of reported cases are concentrated in Ituri province, creating a focal point for response operations.

Officials also report cases in the provinces of North Kivu, South Kivu and Tshopo, indicating geographic spread beyond the epicentre. The high concentration in Ituri has placed exceptional strain on local health services and vaccination teams operating in the region.

High test positivity in Bunia indicates ongoing spread

Laboratory results from Bunia, the capital of Ituri province, show elevated test positivity rates, according to WHO briefings. High positivity is consistent with persistent community transmission and suggests that testing may be capturing only a fraction of actual infections.

Health teams in Bunia report difficulties in rapidly isolating suspected cases and in tracing contacts who may be mobile or reluctant to seek care. These operational challenges increase the risk of further spread to neighbouring districts and provinces.

Containment challenges and response measures under way

Response efforts face multiple hurdles including limited surveillance capacity, under-resourced local clinics, and population movement within and across provincial borders. WHO and DRC health authorities are working to expand testing, accelerate vaccination campaigns, and strengthen contact tracing in hard-hit communities.

International partners and non-governmental organizations are being mobilised to support logistics, laboratory capacity and risk communication. Public health teams are also intensifying community outreach to address distrust, misinformation and barriers to care that hinder rapid case detection.

Implications for regional health security and monitoring

The possibility that the outbreak is substantially larger than reported heightens concerns about containment and the potential for cross-border spread. Regional health officials are urging neighbouring countries to increase surveillance at points of entry and to prepare clinical and laboratory capacities for potential imported cases.
